Comparative usability testing helps a team understand why two designs perform differently before either becomes the default.

Comparative testing moves a design decision from internal preference to observable use. Preference captures what people say they like; performance shows what the interface helped them complete, how much checking it demanded, and where recovery broke down. When those signals disagree, the split is not noise to average away but a clue about what should be revised and tested again.
Decision rule: When results are split, improve the observed weakness before declaring a winner.
On a compact transport monitor, clinicians needed to find settings quickly while moving patients between care areas. The familiar grid menu had worked well in the legacy product, while a proposed scrollable linear list offered a clearer reading order but altered the navigation pattern users were already familiar with.
Internal discussion could not resolve whether familiarity or structure should lead the decision, so we ran a comparative formative usability test with equivalent tasks across both alternatives. The first results were split: task success and preference did not point to the same menu, and observation showed where users missed items or hesitated while scrolling. Instead of reducing the findings to a single winner, the team revised the linear list to reflect the observed breakdowns.
In the second comparison, the updated list achieved higher success rates and stronger ratings than the legacy grid. The test did more than choose a menu; it showed what the newer approach needed before it was ready to replace the familiar one.
Changing the menu structure changes what users recognize, how they scan for functions, where they hesitate, how they recover from a wrong turn, and which use-related risks must be reassessed.
Ship this by defining the decision question and success threshold before the first participant begins.

AI and Comparative Testing
Artificial intelligence (AI) can reduce synthesis effort by aligning task logs, observation notes, ratings, and hesitation points across alternatives. It can flag cases where preference and performance diverge and help the team generate hypotheses about the cause. AI supports evidence work; it does not replace representative users, clinical judgment, statistical reasoning, risk review, or accountable product decisions. Treat AI output as hypotheses, keep contradictory cases visible, and validate every important interpretation against the recordings and source data.
